Please read these Terms of Service ("Terms") carefully before accessing or using the website or enrolling in the subscription services offered by Perimeter Weight and Wellness LLC ("we," "us," or "our"). By accessing our website and utilizing our clinical oversight services, you agree to be bound by these Terms.

IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 IMMEDIATELY OR GO TO THE NEAREST EMERGENCY ROOM. This website, its content, and our subscription services are strictly for elective, non-emergency medical weight management oversight. Our platform does not provide emergency medical care.

1. Description of Services

Perimeter Weight and Wellness LLC provides an online telehealth platform connecting individuals with licensed healthcare practitioners for weight management clinical oversight. The monthly fee covers clinical evaluations, ongoing medical oversight, and administrative access to our medical team.

2. Telehealth Informed Consent

Telehealth involves the delivery of healthcare services using electronic communications. By initiating our services, you understand and agree that:

  • Prescriptions (including GLP-1 medications) are issued solely at the independent clinical discretion of your assigned licensed medical provider. Enrolling in the subscription does not guarantee a prescription will be written.

  • You must provide completely accurate, truthful, and updated medical histories and lab data. Failure to do so may result in the immediate termination of your account without a refund.

3. Geographic Restrictions

You must be a legal resident of, and physically located in, one of our serviced states at the time of your medical consultation. We currently only accept patients located in: Georgia.

4. Subscription Billing, Cancellation, and Refund Policy

  • Automatic Recurring Billing: By enrolling in our monthly program, you explicitly authorize Perimeter Weight and Wellness to charge your payment method on file automatically every 30 days for your recurring subscription rate of $259-339 depending on subscription tier.

  • Cancellation Policy: You may cancel your subscription at any time. To avoid being billed for the next consecutive month, you must either message your provider through the patient portal or submit a written cancellation request to admin@perimeterwellness.com prior to your next billing date.

  • Refunds: Because the subscription covers clinical time, evaluation services, and administrative overhead, refunds are not issued if your prescription order has already been processed.

5. Limitation of Liability

To the maximum extent permitted by applicable law, [Your Business Name], its directors, employees, or affiliated medical providers shall not be held liable for any indirect, incidental, special, or consequential damages resulting from your use of the site or services, or from your use of any medication fulfilled by an independent pharmacy.

6. Governing Law

These Terms and any disputes arising out of your use of this platform shall be governed by and construed in accordance with the laws of the State of Georgia, without regard to its conflict of law principles.
